Dear Parent/Guardian,
We are offering an after-school Extra Support Program for students in 6th through 8th grade who may be struggling with completing assignments, on Mondays and Fridays. This program is designed to provide additional academic support, structured work time, and guidance to help students complete missing or incomplete work and improve their grades.
The program will provide a quiet, supervised environment where students can focus on assignments and receive assistance as needed.
Program Details:
• Open to students in Grades 6–8
• Focus on assignment completion and grade improvement
• Held after school on Mondays and Fridays
• Supervised by the school social worker, Ms. Herrera
Participation is voluntary but strongly encouraged for students who would benefit from additional academic support.
If you would like your child to participate, please complete the information below and return this form to the school.

Hello Conchos Parents,
We are excited to share an important opportunity for our 8th grade students. Southwest Behavioral Health will be providing a prevention program focused on e-cigarette awareness and strategies to reduce vaping among students.
The program will take place on Mondays from 2:00–2:50 p.m. during the school day and is designed to educate students on the risks of e-cigarette use and promote healthy decision-making.
If you do not want your child to participate in this program, please notify the front office or contact me directly using the information listed below. A permission slip will be sent home with your student. If you do not want your student to participate, please return it signed to identify those whom will not participate.
If we do not receive a returned permission slip, your student will participate in the program.
