📝 Annual Parent/Caregiver Survey

Share your experiences and perceptions of Southwest School and the Roosevelt School District to help us understand what’s working well and where we can address barriers to engagement, better support families, and ensure our services are meeting the needs of our community.

The survey is anonymous and available in English and Spanish.

Students whose parents/caregivers complete the survey will receive raffle tickets and 25 PBIS points. The classroom with the highest completion rate wins a pizza party! To ensure your student receives their incentives:

  1. Complete the survey.

  2. Screenshot the final confirmation page.

  3. Share the screenshot with your student's teacher or a school staff member.

Hello Families!
We’re excited to introduce student ID badges this year to help keep our school safe and running smoothly. These badges will make it easier for students to:
• Tap on and off buses for quick, accurate attendance
• Enjoy faster access to school meals
• Be easily recognized as part of our school community
Please remind your child to wear their badge every day. If forgotten, temporary cards are available in the office, and lost badges can be replaced for $5. Thanks so much for your support in helping us create a safe, welcoming environment where every student can thrive!

Dear Families, We’re excited to share an important update that will help us continue creating a safe, efficient, and supportive learning environment for all students! This year, each student will receive a school identification badge. These badges will make daily routines smoother and safer by allowing students to: Scan on and off District transportation for accurate attendance and safety tracking Receive school meals quickly and efficiently Be easily identified as part of our school community during the day and at school events We ask that students wear their ID badge at all times during the school day and while participating in school-sponsored activities. If a student forgets their badge, they can stop by the office in the morning to receive a temporary card, which will be returned at the end of the day. If an ID is lost, please let the school know right away—replacement cards can be issued for a $5 replacement fee (as approved by the Board). We appreciate your support as we roll out this new system designed to keep our students safe and help our school day run smoothly.
